CBC with diff; chest x-Ray; sputum culture - not good; Blood culture - better; Best initial test: chest xRay – - lobar consolidation (typical) - Interstitial infiltrates (not typical) Or CT or MRI Etiology: sputum gram stain and sputum culture Ddx with bronchitis: SOB, high fever, abnormal x-ray Dullness to percussion (with effusion) Consolidation in lungs: bronchial sounds and egophony Sputum color doesn’t help! Atypical pneumonia: Mycoplasma, Chlamydophila, Legionella, Coxiella, Viruses Blood culture is positive: S. pneumonia
